Acknowledgement of Risk
I, the undersigned parent or legal guardian, acknowledge that roller hockey is a fast‑paced physical sport that involves skating, sticks, pucks/balls, and physical contact. I understand that participation may involve risks including, but not limited to, falls, collisions, cuts, bruises, sprains, fractures, or other injuries.
I confirm that I have considered these risks and voluntarily consent to my child’s participation in roller hockey activities, including training sessions, matches, tournaments, and related events.
Mandatory Safety Equipment
I understand and agree that appropriate safety equipment must be worn at all times during roller hockey activities.
At a minimum, this includes:
- Helmet (properly fitted and fastened)
- Gloves
- Shin pads
I acknowledge that additional protective equipment (such as elbow pads, knee pads, mouth guards, and protective cups) is strongly recommended to further reduce the risk of injury.
I accept that failure to wear the minimum required safety equipment may result in my child being unable to participate.
Medical Fitness & Emergency Treatment (UK Consent)
I confirm that my child is medically fit to participate in roller hockey activities and that any relevant medical conditions, allergies, or special requirements have been disclosed to the coaching staff.
In the event of an accident or medical emergency, I authorise coaches, officials, or organisers to seek and obtain appropriate medical treatment for my child if I cannot be contacted immediately, including treatment provided by the NHS or emergency services.
Assumption of Responsibility & Waiver
To the fullest extent permitted under UK law, I agree to accept responsibility for all risks associated with my child’s participation in roller hockey activities.
I understand that, under the Unfair Contract Terms Act 1977 and other applicable legislation, liability for death or personal injury resulting from negligence cannot be excluded. Subject to this, I agree to release and hold harmless the club, league, coaches, volunteers, officials, and venue operators from liability for injury, loss, or damage arising from participation in roller hockey activities.
Volunteer-Led Organisation & Coaching Disclaimer
I acknowledge and understand that the team and club are run entirely by volunteers.
I further understand that coaching sessions may be delivered by volunteers who do not hold formal coaching qualifications. I accept that instruction is provided to the best of the volunteers’ abilities, with an emphasis on enjoyment, basic skill development, and safety, rather than professional or elite-level coaching.
I agree that participation is conditional upon this understanding and accept the inherent risks associated with a volunteer-led, community sports environment.
Code of Conduct & Supervision
I understand that my child is expected to follow the instructions of coaches and officials, play safely, and behave in a respectful manner at all times. I acknowledge that reasonable supervision will be provided by volunteers, but continuous one-to-one supervision cannot be guaranteed.
